  6. Would you like to remain anonymous? Employees can select either ‘Yes’ or ‘No’ to indicate whether they want to remain anonymous.

  7. If no, please provide your name: This question is displayed only if the employee selects ‘No’ in the previous question. It allows employees to provide their name.
